Side Effects, Dosage, Price, and More

Janumet and Janumet XR can cause mild or severe side effects. The following lists contain some of the main side effects that can occur when taking Janumet or Janumet XR. These lists do not include all possible side effects.

Mild side effects

Mild side effects* of Janumet and Janumet XR may include:

Most of these side effects may disappear within a few days or weeks. However, if they get worse or don’t go away, talk to your doctor or pharmacist.

Serious side effects

Serious side effects from Janumet and Janumet XR are not common, but they can occur. Call your doctor right away if you experience any serious side effects. Call 911 or your local emergency number if your symptoms are life-threatening or you think you have a medical emergency.

Serious side effects and their symptoms may include:

  • Bullous pemphigoid (a rare but potentially serious skin reaction).* Symptoms may include:
    • painful blisters on certain areas of the body, such as the palms of the hands, abdomen, groin, and soles of the feet
    • redness or discoloration around the affected area
  • Hypoglycemia (low blood sugar).** Symptoms may include:
  • Pancreatitis (inflammation of the pancreas).* Symptoms may include:
    • diarrhea, nausea or vomiting
  • Severe joint pain.* Symptoms may include:
    • swelling, tenderness, and stiffness around the affected joint
  • Lactic acidosis (dangerous buildup of lactic acid in your body).†
  • Allergic reaction.‡
  • Heart problems.‡
  • Kidney problems.‡

* This side effect has not been reported in clinical trials of Janumet and Janumet XR. It was reported after the drugs were made available.

Details of side effects

Here is some information about some side effects this drug may cause.

Lactic acidosis

Lactic acidosis is a possible side effect of Janumet and Janumet XR. In rare cases, this side effect can be fatal. With lactic acidosis, there is a dangerous buildup of lactic acid in your body. This condition is a serious medical emergency that requires immediate treatment.

Because of this risk, Janumet and Janumet XR have a framed warning for lactic acidosis. This is the most serious warning from the FDA. A boxed warning alerts doctors and patients to potentially dangerous drug effects.

Lactic acidosis has not been reported as a side effect in clinical trials of Janumet and Janumet XR. However, cases of lactic acidosis have been reported since metformin became available. (Metformin is one of the active ingredients in Janumet and Janumet XR.)

You may have a higher risk of lactic acidosis with Janumet and Janumet XR if you have certain conditions. These include kidney and liver problems. Your risk may also be higher if you drink large amounts of alcohol or are 65 or older.

Symptoms of lactic acidosis include:

If you have symptoms of lactic acidosis, stop taking Janumet and Janumet XR and call your doctor right away. However, if your symptoms are life-threatening, you should immediately call 911 or your local emergency number.

Heart problems

Heart problems are possible with Janumet and Janumet XR. This side effect has not been reported in clinical trials of Janumet and Janumet XR. However, some heart problems have been reported in studies with other dipeptidyl peptidase-4 (DPP-4) inhibitors. (Sitagliptinone of the active ingredients in Janumet and Janumet XR, is a DPP-4 inhibitor.)

Specifically, cardiac arrest has been reported in studies with other DPP-4 inhibitors. Symptoms of heart failure can include:

Be sure to tell your doctor if you have heart failure before starting treatment with Janumet or Janumet XR. These drugs can make your condition worse. Your doctor can advise you if it is safe to take Janumet or Janumet XR.

Tell your doctor right away if you have any symptoms of heart failure while taking Janumet or Janumet XR. He will likely prescribe a different medication to treat your condition.

Kidney problems

Taking Janumet and Janumet XR can cause kidney problems, such as kidney failure. Kidney problems were not reported as side effects in clinical trials of Janumet and Janumet XR. However, this side effect has been reported since the drugs became available.

Symptoms of kidney problems can include:

Your doctor will order kidney function tests before you start treatment with Janumet or Janumet XR. And they will likely check your kidney function throughout your treatment with the drug.

Keep in mind that kidney problems may increase your risk of lactic acidosis* as a side effect of Janumet or Janumet XR. (With lactic acidosis, there is a dangerous buildup of lactic acid in your body.) Because of this risk, it is important to tell your doctor if you have symptoms of kidney problems while taking Janumet or Janumet XR.

If you develop kidney problems while taking Janumet or Janumet XR, your doctor will likely tell you to stop taking the medicine. They may prescribe a different treatment for your condition.

Allergic reaction

As with most medicines, Janumet and Janumet XR may cause an allergic reaction in some people. This side effect has not been reported in clinical trials of Janumet and Janumet XR. However, allergic reactions have been reported in people taking sitagliptin after it is made available. (Sitagliptin is one of the active ingredients in Janumet and Janumet XR.)

Symptoms of a mild allergic reaction may include:

A more serious allergic reaction is rare but possible. Symptoms of a severe allergic reaction may include:

  • swelling under the skin, usually in the eyelids, lips, hands, or feet
  • swelling of the tongue, mouth or throat
  • difficulty breathing

Call your doctor right away if you have an allergic reaction to Janumet or Janumet XR, as the reaction could become serious. Call 911 or your local emergency number if your symptoms are life-threatening or you think you have a medical emergency.
