Side Effects, Uses, Dosage and More

Like most medications, Topamax can cause mild to severe side effects. The lists below describe some of the more common side effects it can cause. These lists do not include all possible side effects.

Keep in mind that the side effects of a medicine can depend on:

  • your age
  • other health problems you have
  • other medicines you are taking

Your doctor or pharmacist can tell you more about the potential side effects of Topamax. They can also suggest ways to help reduce side effects.

Mild side effects

Mild Topamax side effects that have been reported include:

  • loss of appetite
  • stomach ache
  • changes in the taste of food
  • diarrhea
  • dizziness
  • feel nervous
  • insomnia (sleep disturbances)
  • memory problems
  • nausea
  • slow reactions
  • speech problems
  • tingling in the arms or legs
  • weightloss
  • feel like sleeping*
  • hair loss*

Mild side effects of many medications can disappear within days to weeks. But if they become bothersome to you, talk to your doctor or pharmacist.

Serious side effects

Serious Topamax side effects can occur, but they are not common. If you have serious side effects from Topamax, call your doctor right away. But if you think you have a medical emergency, you should call 911 or your local emergency number.

Mood and personality changes

Topamax treatment can cause mood and personality changes. You may notice changes in the way you behave, feel, interact with others, and react. For example, treatment with Topamax can cause:

Although these side effects are serious, they have been reported very rarely in people taking Topamax.

It is important to note that all anticonvulsant medications carry a risk of suicidal thoughts. (Topamax is an anticonvulsant drug.) Additionally, epilepsy itself is linked to an increased risk of suicidal thoughts and behaviors. It can be difficult to tell if this side effect is due to Topamax, the condition being treated, or both.

What might help

Here are some tips to help you manage your mood, thoughts, and behaviors while taking Topamax:

  • Pay close attention to any changes, especially sudden changes, in your behaviors, feelings, mood, and thoughts. Keeping a journal can help you track and notice any changes or patterns. Talk to your doctor if you notice any changes that bother you.
  • Keep all appointments with your doctor. It is very important to see them regularly while you are taking Topamax. This helps your doctor monitor your behaviors and mood during treatment.
  • Talk to your doctor if you want to stop taking Topamax. You should not stop treatment unless you have spoken with them first. If you suddenly stop taking Topamax, serious problems may occur, including worsening of seizures.

Tell your doctor right away if you notice any of the following symptoms while taking Topamax:

  • aggressive or violent behavior
  • feeling agitated, irritated, or agitated
  • new or worsened feelings of anxiety or depression
  • panic attacks
  • thoughts about suicide or death
  • insomnia (sleep disturbances)
  • other unusual changes in your behaviors or mood

If you have mood or personality changes with Topamax, you and your doctor can decide the best course of action. For example, they may suggest changing your dose. Your doctor might also recommend ways to treat your side effects. Or they may advise you to stop taking Topamax and switch to another medicine.


If you think someone is at immediate risk of harming themselves or another person:

  • Dial 911 or your local emergency number.
  • Stay with the person until help arrives.
  • Remove any guns, knives, drugs, or other objects that could cause damage.
  • Listen, but don’t judge, argue, threaten or yell.

Feel like sleeping

Topamax treatment may cause drowsiness as a side effect. This was one of the most common side effects reported by people taking Topamax to treat epilepsy in studies. Drowsiness was less common in people who took Topamax to help prevent migraine in these studies.

People often have to stop taking Topamax because of the drowsiness the drug can cause.

The risk of drowsiness from Topamax increases with higher doses of the drug.

What might help

Because of the risk of drowsiness with Topamax, it is important to avoid certain activities until you know how the drug will affect you. These activities include driving and using machines.

The drowsiness caused by Topamax may go away on its own. Talk to your doctor if you have any drowsiness that bothers you or doesn’t go away. They may lower your dose of Topamax. Or they may recommend a different medication.

Hair loss

Hair loss is possible with Topamax. But this side effect was not common in the drug studies in people being treated for epilepsy. Hair loss has not been reported in people who have taken the drug to help prevent migraine.

What might help

Talk to your doctor if you have any hair loss that bothers you while taking Topamax. They may be able to suggest treatment for this side effect. But they may also recommend switching to a drug other than Topamax.

Allergic reaction

Some people may have an allergic reaction to Topamax.

Symptoms of a mild allergic reaction may include:

  • itchy skin
  • itching
  • rinsing (temporary warmth, redness or deepening of skin color)

A more serious allergic reaction is rare but possible. Symptoms of a severe allergic reaction can include swelling under the skin, usually on the eyelids, lips, hands or feet. They can also include swelling of the tongue, mouth, or throat, which can lead to difficulty breathing.

Call your doctor right away if you have an allergic reaction to Topamax. But if you think you have a medical emergency, call 911 or your local emergency number.
